On March 31, 2014 Patrolman David W. Smith Jr. responded to a disturbance call on Harrison Street in Johnson City, NY. Upon arrival on scene, a male subject immediately attacked Patrolman Smith as he was attempting to exit his marked patrol car. The male was able to disarm Patrolman Smith of his duty weapon, and ultimately shot Patrolman Smith two times, with these shots proving to be fatal.

A second officer arrived on scene just as the fatal shots were fired, and he was able to return fire striking the male multiple times, ending the threat. The second officer was not injured.

Patrolman David W. Smith Jr. served with the Johnson City Police Department for 18 1/2 years.
